The Japanese Society of Psychiatry and Neurology and Japan GI Society 5th-edition guideline lists puberty blockers (GnRH agonists) from Tanner 2 for adolescents, with parental consent. Opposite-sex hormones are in principle from 18, with a 15-and-over exception after a year of team observation. The Act on Special Cases in Handling Gender Status lets a family court change legal sex from age 18.
